How Do I Become a Member?

First, make sure you are eligible.

• Are you HIV +?
• Do you have Medicaid*?
• Do you live or work in Manhattan, Brooklyn, or the Bronx?

*Remember, persons who also have Medicare (SSD) or Medicaid Spend Down are not eligible to join Amida Care.

Next, before you apply to Amida Care, we want you to be sure that this is the right Plan for you. We will talk with you by phone or in person to explain what Amida Care is and how it will affect your health care. We want to be sure that you get all your questions answered.

To join Amida Care by phone, contact NY Medicaid CHOICE at 1-800-505-5678.

If you would prefer to talk with an Amida Care Enrollment Specialist before you enroll, you can either:

1. Call Amida Care’s toll free 1-800-556-0689
2. Or print out a hard copy of the Membership Inquiry Form send it to us, and we will contact you to arrange an appointment at a location that’s convenient for you!
3. Or complete the Online Membership Inquiry so we can call you to arrange the appointment!

Remember: Joining an HIV SNP is voluntary. There are other SNPs to choose from.
